Weird tab in table of contents – MS Word

Neither my table of contents, hyperlink, or heading styles have a tab at 4 cm, however whenever I have a short heading (three-four letters long) in the table of contents, there is a tab at 4cm and the page number that is supposed to be on the right appears at the 4cm tab.

So I have to manually remove the tabs from the ruler bar every time I update the TOC.

Can anyone please help?
