making the invoice

I have 2 table in 1 sheet.

second table.
Good / 1 2 3 <-[custommerPriceType ] 1 A / 5 7 9 <-- in $ 2 B / 7 9 12 <-- in $ 3 c / 9 12 15 <-- in $ .. . .. .. .. I need to make invoice, whit Validation custommer name & type_custommer_price in onepiece ?
