Outlook Custom Form

I have created a new contacts form and have converted the previous contacts to the new form but I can’t get the Outlook Contacts Folder to recognize the new form that is to be used for new Contacts.

I have right clicked Properties and “When posting use – ” but the new form name doesn’t appear to be available. Somehow when I published the form, it does not appear in “Personal Forms Library” and when I try to republish I get a “Unable to successfully publish the form due to a MAPI error.”

Any suggestions?
