Outlook Forms

I’ve created an Outlook Form for reporting absences. The email is addressed to me, with a cc to our bookkeeper. The customized form is on page 2 of the email form. When I test the form it gets sent to both me and the bookkeeper, but the person’s input on page 2 doesn’t come through in the objects.
