Printing Outlook Form

I have developed an Outlook email form that allows salespeople to request that thepurchasing department find product that is not currenlty in stock. All clients use Outlook 2000 and the form will be posted to a the organization form library on Exch 5.5. The form is working except for when a person prints the form the data in the upper part of the form does not print -how do I get this to print? Thanks.
