Special Form in Word

I need some help from a Word 2003 expert. My wife would like to create a form in Word and then email the form to receipients who would then fill out this form with their responses and automatically email it back to the sender.
We’ve experimented with the “reply with changes” option in Outlook but not sure if this will work right since some of the recipients use Outlook Express instead. Any suggestions would be greatly appreciated.
