System Administration Password Policy

I’d be interested in feedback from people on what policy’s they have in place for System Administrators and System passwords.

How do you control which administrator is doing what when multiple administrators have access to a system? If you have limited access to certain systems by certain administrators what happens if they are not contactable and someone else is required to administrate how do you control the password distribution?
