Training Survey

I am a software trainer for my agency and I am trying to develop a training survey for our users. I want to get an idea of what types of training classes our users might like to have. We have the MS Office Suite (currently 97, but soon to upgrade to XP).I am stumped as to what questions to ask so I can get the response I am looking for. Has anyone created a survey like this before. If you have would you be willing to share an example or two with me, just to give me a jump start.
