Trouble sending custom forms in Outlook

I have a user in my office that is trying to send a custom form based on the ipm.note form. When he sends it out, the recipient gets the email, but the body of the email is now shrunk down to 1 line with a drag-down box replacing the body. Has anyone dealt with this before? How do I fix this problem? Help would be appreciated.
