Using Forms in Outlook

Using outlook 2002 with win XP. Is it possible to create a Form in Outlook then for the Form to pop up when a particular users name is entered in the “to” field.
We would like to use a template with particular recipients in outlook and though a Form would be the best way. Let me know if this is possible?.
