Evaluating a Buisnesses IT needs

I was wondering if anybody knows of a good form to use to evaluate a businesses current hardware, software and future business goals. I’m looking for something to use, when meeting with a company to evaluate what they could be doing differently and what they’ll need in the future. Thanks.
