Fatal Exception in VXD VWIN32(04)

When dialing into the state from a billing software to verify patient eligibility information I get “A Fatal Exception OE has occurred at 0028:C0275C87 in VXD VWIN32(04) + 00003E63”. I can dial 2 times into the system just fine but on the third time I always get this error message. I then have to reboot in order to be able to dial again. This started happening after doing an upgrade to the software. The funny thing is that I have several machines running the same software and it only happens on one machine. The modem seems to stop responding after the error until I reboot the PC. Any suggestions will be appreciated.
