IPF in unknown module at 0000:*

Lately I’ve been running into IPFs in various programs in module unknown at various addresses that start with 0000:*. This error occurss on win95 and 98. I haven’t had any luck at Microsoft’s Knowledge Base. Isn’t this at Windows driver issue? Any other ideas where to get info?
