Migrating/Consolidating Servers

Need to consolidate NT servers down to fewer servers. Means I migrate applications to new servers. How to determine applications/ components/APIs/DLL’s, databases, etc. that will be affected by move/consolidation ? Any known scripts, tools, etc. available to do this ? Also, to ID embedded links, hard-coded server names, IP addresses, etc.
