Outlook Contact Form

In outlook I tried to modified the Contact Form and it work well if assigned this new form name for the folder, but if I import data from *.csv file, outlook use the original Outlook form instead of mine, so is there anybody know how to get that thing working or is there a way to modify the original Outlook form??
