Run command /wo Logo Key being pressed

I have seen several of my clients PCs pull up various windows hotkeys without the Windows Logo Button being pressed.

For example Logo+R=Run Dialog, but the Windows Logo isn’t being pressed and the
accessibility options aren’t loaded. So it can’t be sticky keys or the other functions.

What causes this to happen. And how do i avoid it/turn it off when it does happen.
