Termination of wires on RJ45 jack

how do I connect(do termination) on the wires on RJ45 jack. i mean how do i configure the wires from the patch cable on the jack. i’ve done it once but I’ve forgotten. the jack is connected to patch panel by a patch cable and using a cross cable to connect to d hub/switch. thanx
