Very Clean HTML

Exporting Files from QuarkXpress or Acrobat Writer plugins to HTML generates too much useless code, I need to have very clean HTML without losing the layout( I already tried Dreamweaver MX, and it’s not enough); any help will be awarded with a great place in heaven.
