Windows 98 SE changing icons of files.

Does anyone know how to fix or the reason why Windows will change the file type icon to something different. When I use my computer, Windows will sometimes change the file icon to some other icon. The file will still be the right extension, but the icon associated with that file type will change to another type. Any ideas?
