Access 2000 Report Formatting

I am trying to build a report/PO Letter from one of my forms where the purchasing agent enters some information. One of the fields in the form is a true/false checkbox. If the box is checked true, then a PO Letter needs to be generated which would include the suppliers name, address, etc. (The supplier is selected on the same form by using a combo box.) The report/PO Letter also needs to continue looking at the form for additional true conditions and include those items on the same PO Letter. Also, before the preview of this report oens, I need a pop up bow to prompt the purchasing agent for any additional comments he would like to include in the PO Letter. Can anyone please help me in writing the code for this function? Also, please be patient with me because I am a newbie at this, and pretty much need someone to guide me step by step on how to do this. If you would like some screen shots of the form, please let me know and I will put them up on a web site.
