Creating Security Permissions Database

Hello,

Is anyone familiar with creating or building an Attestation Repository or Permissions Database for business applications that require accounts to be permissions? I am looking for any templates that are related to it but cannot find any.

An example would be something that includes the following:

username (JSmith), full name (John Smith), application (Active Directory), permission level (Domain User)
