How do I get this dialog box in Word 2003?

I was working with a text box in MS Word 2003. I wanted to include a grpghic. I don’t know what I did, but I got up a window that contained a choice of graphics with associated text. Almost like this was supposed to create a logo. Now I’m trying to duplicate what I did, and I can’t find it. Has anyone seen this kind of dialog box?
