How to implement a CMDB

I have read a lot of documentation about general aspects of the CMDB implementation but there’s no specific information or any guide about the CMDB structure.
I have the ITIL foundations certificate, I read almost everything about the CMDB, I know the philosophy and it’s very interesting. But…
Has anybody a CMDB implemented? What software do you use? How is the general structure?
