Performance Testing

I am supposed to be performance testing a database-driven Web site. I have done much research on the topic and have more-or-less developed my approach.

However, I need an outline for the test strategy/plan document. I understand the approach and the various steps to take to actually define the scope, conduct the test, analyze the results, etc., but I need a way to articulate this.
