query for List boxes in excel

I have a database of five columns
A,B,C,D and E

A is the site say 1 2 3……250
B is the data entity under A
C is B’s description
D is the data which B is linked to
E is D’s description

How to write a code to retrieve the CDE columns if i select A and B in the form screen
