Questioner for IT Service

I recently hired an office administrator that will be handling by schduele. I am a tech that has small/meduim size businesses that we support all of their IT Needs. I am looking for a questioner doc or something that has questions that she can ask that helps her dispatch me the most efficient way, plus provide me with the most accurate client information.. Any suggestions would be great.
