Using a Check Box to Inactivate all fields on my form

I have a declined check box on my form which controls a single record at a time, and I want this check box when it is checked to deactivate all the fields on the form and have a password assigned to uncheck it. Secondly, I do not want the deactivation to carry over from record to record unless the box is checked on the individual record. Please help…
