What does a final consulting report REALLY needs

At the conclusion of a brief training project (5-6 classes, minimal investment dollars-wise) what does the final report really need to cover? Does any have a sample final report to share? I’m thinking 1-2 pages maximum.
I’d appreciate any samples or report headings other managers would like to share that they would definitely like to see covered.
