Win 98 driver for a modem

I don’t know the make or modle of the modem, but it’s got an FCC ID tag of 52PT-3511.
A couple of the chips have Rockwell 97 on them. Along with the ID numbers, RLVDLPF/SP, 9844 B70976.2, R6785-61, 11229-14 and 9823 B64537.2
I’ve checked the FCC web page but the ID tag doesn’t seem to fit any of the search engines.
