DATA LOST ON FLOPPY

Has anyone ever saved a document to a floppy disk, then when you open it, all you see is a bunch of blank squares. The data is not readable and ususally lost. I know this happens sometimes because of different formats, but is there any way to restore the data? Thanks for the input.
