Looking for IT Documentation Template

I’m currently taking an IT class and one of the things the instructor mention was to keep proper documentation of all your troubleshooting, changes, and service agreements. He recommended finding some outlines or templates online, but most of what I found was either very old or not beginner friendly.

I want to start practicing on my own and start an IT binder. Does anyone have any recommendations about how to start one or knows where I can find some?
