OWA 2003 login issues

I am having 2 issues with owa.

When logging into owa we must use http://mail.domain.com/exchange/username. I know that you should not have to include the /username.

The second is when the un/pwd box comes up we have to put domain/un or full email address.

We are running exchange 2k3 enterprise not using forms and not using ISA.
