Project Sign-off Agreement

Hi,

I was wondering if anyone out there knows of a good template for project sign-off. As a consultant, I need document that will be more then a typical documents out there with tables and list of issues.

I need a document that contains legal language for:

– client accepting the delivery of the project
– being entitled to X number of days of free bug fixing
