Responsibility of post application bugs?

Who should share the responsibility of postproduction bugs?
QA testers or Developers or project managers. We know that ultimately project management is responsible but testers are most vulnerable compared to everybody else in the project. Developers and management hold Q.A testers responsible for bugs, which creep in a system after production. They are always blamed for the poor work done by others.
