Why Outlook 2003 forms do not work

This irritating mystery is driving me nuts!
I followed all the neccessary steps that Microsoft dictates & the form in Outlook is not working as I want to. When I send it to a user, it appears blank & even when the user hits reply on the same message, it comes back blank!

I need help from professionals who could solve this puzzle for me, please.
