can send fax on server 2003 when i use a cover page

hi all, was wondering if you could help. i have set up a shared fax printer from server 2003 and shared it with all the clients 9 in total. the fax recieves fine and will send no problem until i select a cover sheet templte from word. then it does not even appear i the fax cue on the server. any help would be great.
