CAT5 wiring

What is the industry standard when connecting CAT5 wiring to wall outlets on one end and to a hub on the other end? What colour coding is followed? I recently ran into a problem trying to figure out and had to use a multimeter to check the connections and still I am not very clear.
Is there any publication on the net which I can read and/or download?
