Creating shortcut in all Outlook Client, When a template is newly published

Hi,

I like to know how to add a new menu in menubar for a newly published Custom template in a Exchange server. ie. A new custom Form is designed in the name of Leave Request and published in Orgn Form Libaray in Exchange Server. I like to have a menu like File -> New -> Leave Request. On Click of the Leave Request menu, the custom form published in Exchange server needs to be opened. Where should i need to code for the adding the custom form in the menubar, when publishing it in the Exchange server
