Document window opens randomly

Running Vista Home Premium

The document window randomly opens. I have scanned using norton, spy-bot, no help. Searched the web for advise, no help.
Looked for hidden windows….none of this has worked. I am at my wits end on this one.

Any help on this would be greatly appreciated.
