Enter Primary Disk Password

Whenever i start my laptop, it asks to enter the Primary disk password, which actually i have never set. Many times laptop starts without ny problem and in the middle of my work, i face the same primary disk password problem.Got it formatted but not solved.
Plz tell me what can i do now..?
