Fujitsu MPG3204AT Hard drive failures?

I am looking for anyone who has had a Fujitsu 20.4Gb (MPG3204AT) fail within 18 months of the purchase date. As a technician, I have seen a abnormally high failure rate on these particular drives.

If you are kind enough to respond, please answer the following questions:
1. What is the date of manufacture?
2. What is the serial number?
3. What is the model?
4. How long did you own it before it failed?
5. In your opinion, what was the cause of the failure?
6. Was you drive replaced under warranty by the PC manufacturer or Fujitsu?
7. Was any critical data lost?
8. If so was it sent away for data recovery.
9. Was data recovery sucessful?

All responses will be compiled and forwarded to a class action lawyer.
