handeld scanners

Has anyone had any experiance sucess with scanning information off of files with some type of handheld scanner and importing that information into a format suitable for download to a spreadsheet? For instance Medical Records information, patient ID, Date of Birth Med Rec # etc. what type of scanner do you use?
