HELP!! Custom document size missing

I up-graded a workstation to 2000 Pro. Now, the custom form created in WORD (7×7) doesn’t print properly to a network HP laserjet. As Admin, I created the form as usual in the Printers folder using File>Server Properties>Create new form. However, When setting up the actual print job, The new form doesn’t appear in the Printer properties as an option in Page/Output. Need to print this receipt….Must help appreciated!!
