Losing Folder Permissions

For the past 6 months, one of my file servers has been losing all permissions on certain files and folders. I resolve it by taking ownership of the item and resetting the permissions manually. I have been searching online for a solution for the past 4 months with no such luck. So now I reach out to the IT community for help.
