MS Outlook 2003 missing name header while printing

Hi guys, I got this problem recently with my Outlook 2003. Previously i am using a IMAP account for my 2003 but recently i had add another MS Exchange Server account into it and this where the problem start.

Before whenever i print the my email, it will always show my name on top of the printout, but after adding the MS Exchange Server account, it just disappeared. Tried alots of ways but it just would not work. Can someone help me on this?
