Need help with new site development

Our business wants to open another site and I need some help developing the project.

The new site will have a few offices that are not in the same building and they?ll be 20 miles far from the headquarters. There will be around 15 computers. My questions are:

– What equipment do I need to get this done (switch, router, etc.)?
– Should we have a T1 line or DSL is fine, too? (Budget always a problem)
– Is it worth it to have a DC on the new site or have user authenticate to main office?
– Should I put services like DHCP, DNS, print, file on the remote site server?
– The users at the new site need to access a HIPAA compliant database at the main office. Should I have them connect through SSL VPN to the main office or put the database on their site, too?
– Should I pull drops for these offices and put a switch or a hub in every office or have them wireless. There will be around 5 tiny offices on the new site, so is it worth it have them wireless connected or go with the regular cabling stuff?

Any help would be very appreciated.
