Need suggestions for Disclaimer

We have installed a separate VLAN public internet access at our office for the patients and case managers to use the Internet without being on our network. We need to come up with some type of disclaimer or acceptable usage blurb to post at the Check In desk and on our information sheet. I was wondering if you folks have any suggestions or if there might be a generic type of blurb to use as a starting point. Thanks for your suggestions.
