NT Dump

After installing Win NT 4.0 on a working system and working with it for a few times, I notice that the installation gives me a dump stating some message that looks like xxxx_xxxx_xxxx_xxx and so on and refuses to work giving a message that we need to start with /crashdebug option. i am unable to do so. please guide me to the actual problem and the solution.
