PCI IDE Cards

Chas has often offered cards for IDE control on older systems for a solution in the $20-30 range, I have a paired question 1) what do you recommend?
2) do you still use the on board ide controllers for cdrom/dvd/cdrw or do youneed to disable them and just use the card?
