Preparing Test Plan

I am testing an application that is to be used for collaboration and training and which has a lot of impact on the LAN and WAN as it has audio features.
Besides testing the features of the product I want to know how to prepare a Test Plan,
conductthe tests, document the findings and then finally prepare the Report.
